INTAKE AND TREATMENT PROCESS

Policy

In order to maintain the safety, harmony and efficacy of the Recovery Works program accepts individuals who are appropriate for the outpatient level of care and has established screening criteria to ensure individuals likely to need a higher level of care are referred to providers who provide such services. This screening done formally or informally, by telephone or in person does not represent a full assessment of the level of care needed but should exclude those for whom a higher level of care would be needed while including those for whom the level of care provided by Recovery Works is appropriate.

Diagnostic assessment of the level of care needed will be completed as part of the psychosocial assessment, however if exclusion criteria present themselves during the intake process the individual may be transferred to a higher level of care before a full assessment is made.

A provisional assessment for level of care will be made by a Physician or other Licensed professional allowed by law to make such assessment and will include both a provisional diagnosis and an initial ASAM Level of Care.

If excluded by screening criteria initially or a need for a higher level of care is discovered, such referrals to other program will be documented in a Referral Log by the person making the exclusion or referral.

Admission Exclusion Criteria

Exclusion criteria are present for candidates requiring a higher level of care. Such individuals are referred to facilities offering a higher intensity of service according to their needs:

  • Individuals exhibiting signs of potentially medically dangerous physical withdrawal may require a higher level of care. Such individuals must be first medically cleared before admission to the program.

  • Individuals with complicated or protracted withdrawal from sedative-hypnotics (e.g., alcohol, barbiturates, benzodiazepines, other sedative hypnotics), that could be exacerbated by autonomic nervous system hyperarousal, use, or hazardous use of medications that potentiate the drug from which the patient is withdrawing.

  • Individuals who have not completed the time period for withdrawal detoxification from sedative- hypnotics or alcohol and who have not been otherwise medically cleared by a physician before admission to the program.

  • Patients who are incapacitated by their withdrawal or intoxication so as to need 24-hour nursing or medical monitoring.

  • Patients lacking adequate behavioral control to be able to benefit from the program.

  • Clients with behavioral problems involving violence or a history of violent outbreaks.

  • Medical problems either related or unrelated to addiction and requiring a higher level of may constitute exclusion criteria for admission.

  • Any individual who presents with any medical condition, intoxication, or disease which would require immediate medical attention must obtain medical clearance before bringing the Recovery Works program.

  • Pregnant females must receive medical clearance prior to participation in the Recovery Works Program.

Procedure

  1. Potential Clients will be contacted by an Intake Coordinator and the Recovery Works program and it’s services will be described.

  2. Potential Client will be screened to determine the existence and severity of a drug or alcohol problem.

  3. The need for inpatient medical detoxification services will be discussed and any exclusion criteria present will be reviewed and cleared by the Medical Director before the Potential Client may be accepted into the program.

  4. At intake an initial Physical Assessment with be performed by Medical Staff qualified to provide such services and a full Physical Assessment will be completed within 24 hours of intake. Such Assessment will include screens for TB, HIV, and other sexually transmitted diseases.

  5. An initial urine sample will be obtained and screened by a CLIA waived test.

  6. Within 24 hours of admission, unless client is unable to hear and respond to requests, an Orientation to the program will be completed by Recovery Works staff. This Orientation in any event must be completed with 72 hours.

  7. Within 10 days or Admission a Psycho-Social Assessment will be completed and an initial treatment plan created.

  8. During the first 30 days Client will have input on a Multidisciplinary Master Treatment Plan which will be updated every 30 days as a result of continued evaluation of treatment.

  9. Prior to discharge Recovery Works will develop a Discharge Summary and Aftercare Plan.
