POLICY
Recovery Works does not provide pharmacy services and medications are not provided by the facility. Medications prescribed by a Recovery Works staff physician must be documented in the client medical record and appropriate follow-up care must be provided and documented by the prescribing physician.
PURPOSE
Many of the persons attending Recovery Works may require medications in connection with their treatment. Successful treatment depends upon the safety and accuracy of assessing and prescribing of the self-administration of medications.
PROCEDURE
Medications are for self-administration only and no pharmacy services are provided by Recovery Works.
The risks, benefits and side effects associated with the use of any medication prescribed by A Recovery Works physician is fully explained to the client and documented in the client chart by the prescribing physician.
The physician will ensure a current list of medications and supplements used by the client is recorded in the client’s medical record and updated prior to any prescription being made.
A record is kept in the client record of all medications prescribed by a staff physician. This record will include:
Name of Drug
Reason/rationale for use
Dose, route and frequency of self-administration
Number of refills
Signature of physician
Client response to medications
Follow up appointments will be made according to the level of care:
Ambulatory Detox – Daily follow up.
PHP – Follow up appointment will be made weekly.
IOP – Follow up appointments will be made no less than every 3 weeks.
OP – Follow up appointment will be made monthly.
Follow up appointments will document:
Any changes in dosage/route and frequency.
Client response to the medications.
Any adverse reactions to the medications.
