INTAKE ASSESSMENT
1. Staff
All patients admitted to the program are evaluated by staff qualified by education, training, and experience, to be knowledgeable about addiction treatment, and who are licensed/certified if required by the state practice acts to perform or coordinate the provision of such assessments.
2. The assessments are individualized, and comprehensive with respect to inclusion of a biopsychosocial assessment of the patient’s disorder, and assessment needs:
• Physical
• Emotional
• Behavioral
• Social
• Recreational
• Educational
3. This assessment is used to confirm the appropriateness of placement at Level 3.5, and to help guide provision for the needs, and the individualized treatment planning process which will follow, taking into account the patient’s
• Strengths
• Needs
• Abilities
• Preferences, and
• Desired Goals.
4. The program has the discretion to use current clinical information concerning and transitioning a client from another licensed program, licensed hospital, or a state or federal agency, if there has not been a discontinuation of treatment.
