INDIVIDUAL TREATMENT PLANNING
Clients admitted to Recovery Works must have a complete treatment plan developed, implemented, and individualized for each client, in association with its treatment providers which are modified and updated as necessary.
An initial treatment plan will be formulated at the time of admission after assessment (within a minimum of 10 working days).
A comprehensive Master Treatment Plan, developed by a multidisciplinary, team will be completed and signed by the client and Clinical Director within 30 days of admission.
The master treatment plan must also contain sufficient information about the client’s treatment, including:
Description of the client’s problems and needs;
Measurable goals and desired outcomes that are to be attained by the client, which include both
long-term goals and
short-term objectives leading to these goals;
The interventions and services that the program will provide to help the client achieve the individual goals and desired outcomes;
The expected course of treatment; and
Identification of the staff person who will provide treatment or coordinate treatment.
