DISCHARGE SUMMARIES & AFTERCARE PLANS

  1. A discharge summary will be prepared for all patients leaving the program, whether or not they have completed treatment, have been referred to another facility, or are leaving against staff advice.

  2. For patients who leave the program before completing treatment, a discharge summary must be completed within seven (7) working days of the patient leaving the program.

  3. For patients who complete treatment, a discharge summary must be completed within ten (10) working days of the patient leaving the program, and must include an aftercare plan (see 111.8.19.17b).

  4. The discharge summary shall be completed by the staff member with primary responsibility for coordinating and providing care for the patient.

The discharge summary shall include:

  • dates of admission and discharge

  • evaluation upon admission

  • desired outcomes and expectations upon arrival

  • list of services provided

  • desired outcomes and expectations achieved

  • summary of progress towards treatment goals

  • reasons for discharge (if prior to completion of program)

  • final assessment of the patient’s status at discharge

  • aftercare plan

AFTERCARE PLANS

Aftercare plans for continuing services and support shall be developed and completed prior to discharge for patients completing treatment.

The aftercare plan shall be completed by the staff member with the primary responsibility for coordinating and providing care for the patient.

The aftercare plan shall include:

  • a final assessment of the patient’s status at time of discharge,

  • a summary of progress towards treatment goals, and

  • a description of services and support the patient is expected to need following discharge and

  • a description of potential obstacles to overcome in order to maintain a drug free lifestyle.

The patient must participate in aftercare planning, and, if applicable, parents or other responsible parties whenever feasible.
