10 - PERSONNEL RECORDS
Policy
Recovery Works shall maintain a written record for each employee and the administrator.
Each employee record shall include at least the following information:
Basic identifying information including name, address, telephone number, and person(s) to notify in the event of an emergency.
A 10 year employment history or a complete employment history if the person has not worked 10 years.
Records of applicable licenses, health requirements, and educational qualifications as required by these rules.
Date of employment
Job description or statement of responsibility.
Documentation of training and orientation as required by these rules.
Any records of the employee’s performance including annual performance evaluations.
The results of criminal background checks conducted by the program prior to employment, and indicating that the employee has no history of violence or abuse which would pose a risk to clients receiving services.
Procedure
No one will be considered an employee or volunteer of Recovery Works until this procedure is complete.
Previous to the time of hire the potential employee will submit to a national background check and the date of this check will be recorded in a new employee file.
Previous to the time hire or before the potential employee will submit to a drug screen and the results will recorded in the new employee file.
Once these results have been obtained the potential employee may begin orientation if no evidence of a history of violence or abuse which would pose a risk to clients, or evidence of substance use in violation of drug-free workplace standards.
At the time of hire or before employee will furnish a 10 year work history and records of any applicable licenses, health requirements, and educational qualifications.
New employee must complete orientation and sign the orientation completion record before they are considered an employee or volunteer of Recovery Works.
On an annual basis employee will be reviewed as to performance during an Annual Employee Review
